In exchange for my child's participation in programs and use of facilities provided by Recrivals LLC or South Florida Soccer, LLC (collectively, "the Organizations"), I, the undersigned, on behalf of my child, myself, and our heirs, fully release and hold harmless the Organizations and their agents from all claims, including for personal injury or property damage, arising from participation.
Key Acknowledgments:
Behavior: The Organizations reserve the right to cancel registration due to improper behavior by a child or parent.
Non-Parent/Guardian: If signing on behalf of a child for whom I am not the parent/guardian, I confirm I have express permission and agree to indemnify the Organizations for any resulting claims.
Photography: I consent to my child being photographed/videotaped for promotional use by the Organizations.
Emergency Medical Treatment: I consent to emergency medical treatment for my child if needed and agree to be responsible for all associated expenses, holding the Organizations harmless unless due to their gross negligence or willful misconduct.
Assumption of Risk: I understand that sports and recreational activities carry inherent risks of physical injury. I voluntarily accept and assume all risks associated with my child's participation.
Dispute Resolution: Any disputes arising from this waiver will first be attempted to be settled through mediation in Broward County. If unsuccessful, disputes will be resolved by arbitration in Broward County. Both mediation and arbitration are conditions precedent to any litigation. The arbitrator's decision will be final and binding, and parties will share equally the costs of mediation and arbitration fees, and bear their own attorney's fees.
